Ethics Committee Application

All human- and society-centered research, for example empirical and descriptive studies that do not involve any intervention in bodily integrity (scale studies, questionnaires, system and model development studies, reviews of data sources, online surveys, etc.) and human and social research that does not use biological material (blood, urine and other biological fluids and specimens), is reviewed by the IZU Ethics Committee.

Ethics Committee research approval applications are received through the Institute Secretariat for our master's and doctoral students, and through the secretariat of their affiliated Faculty for our academic staff. The application forms and sample petitions are provided below. Forms that are out of date, incomplete or incorrect will not be processed. Detailed information required for submitting your applications correctly and properly, together with the Ethics Committee Application Information Form, is provided below.

Recommendations for Avoiding Common Mistakes in Ethics Committee Applications

APPLICATION DOCUMENTS

  • The Application Petition refers to the Ethics Committee Approval Request petition addressed to the Institute Directorate for students and to the Deanship for academic staff.
  • In the Application Form, all fields to be completed by the applicant must be filled in fully, and every page must be initialed and the last page submitted with a wet signature.The form must be completed in accordance with the guidance given in this section and in the information form. The research title in the Ethics Committee Application Form and the title in your application petition must be identical.
  • Information about the research team must be provided under “Researcher(s) Information”. The application must be submitted by the principal investigator. Any other researchers must be listed in order from the greatest to the least contribution to the research. Please note that presenting persons who are not the principal investigator as such, and/or including persons who have made no contribution to the research, is regarded as an ethical violation.
  • Researcher Ethical Undertaking must be submitted with wet-ink signatures, giving the full name of the applicant at the top and the other researcher/advisor below. Photocopied signatures and unsigned documents are considered invalid.
  • Informed Consent Form The procedures and methods to be applied to the sample, together with the related risks, are communicated to the participants by the researcher through the informed consent form. This form is prepared in order to address any gaps in the participants’ knowledge and to obtain their consent regarding the research and application process.
  • Measurement Instruments, All questionnaires, scales and interview questions planned to be used in the research must be submitted initialled (and approved by the advisor, if there is one).
